Home Care and Palliative Care
Home Care (C-DOM)
Home care services (C-DOM) are part of a broader network designed to provide at-home medical and social-healthcare assistance for individuals in a state of fragility who are unable to receive the necessary treatment outside of their living environment.
Qualified healthcare professionals work alongside family members to provide assistance and support. The aim is to offer assistance to both the patient and their families in order to enhance quality of life, ensure continuity of care during transitions between home and hospital, promote clinical stability and autonomy, and empower families through education and training programs.
Home Palliative Care (UCP-Dom)
Using a multidisciplinary and interdisciplinary team-based approach, palliative care addresses the needs of both patients and their families. It provides relief from symptoms, affirms life, and regards death as a natural process that should neither be hastened nor postponed. It enhances quality of life and can positively influence the course of illness.
Palliative care also incorporates psychological and spiritual aspects of patient support. It helps individuals to live as well as possible as they approach the end of life, while also providing assistance to families in coping with the emotional weight of bereavement.
