Discharge Process

The patient’s discharge is authorized by the attending physician of the Specialized Rehabilitation Unit.
Upon discharge, the patient receives a medical summary report to begiven to their primary care physician. This report provides a clear and structured overview of the diagnostic and therapeutic pathway followed during the hospital stay.

The discharge letter also includes the names and phone numbers of hospital contacts for any post-discharge clinical needs, as well as instructions for scheduling a follow-up visit, if required.

This document serves as a vital link between the hospital and the patient’s future care provider, ensuring continuity of care and supporting the overall process of Health Promotion.
